Lifestyle Therapy & Coaching (“LTC”) would like to inform our clients of a recent data security
incident that may involve their personal information. The privacy and protection of our clients’
information is a matter that we take very seriously, and we have worked quickly to resolve the
On or about June 16, 2017, an electronic form that LTC used to search its client contact
information was inadvertently left unsecured and was therefore viewable on LTC’s website.
LTC became aware of this error on August 13, 2017 and was able to identify and secure the
online form the following day. As a result, some of LTC’s clients’ contact information on file
may have been potentially accessible to third parties from June 16, 2017 to August 14, 2017.
The information that was potentially at risk of being accessed included first and last names,
home addresses, telephone numbers and email addresses. LTC has no reason to believe that
anyone has misused its clients’ contact information.
LTC took steps to address this incident promptly after it was discovered, including removing the
form from its website and undertaking an internal investigation of the matter. LTC is also in the
process of reviewing its internal policies and data management protocols and has implemented
enhanced security measures to help prevent this type of incident from recurring in the future.
LTC is notifying individuals affected by this incident. To date, no indication of inappropriate
use of the information exists.
